Hello,

I took the psychiatry coding class and have a question about problem points, specifically getting 4 problem points for new problem with additional workup. The examples in the class were ordering urine tox screen or blood tests. I saw in an AACAP video that using a rating scale after the initial visit qualifies as additional workup. Is this true? If so, what do you have to write in the note for the initial visit? "will order BDI scale in followup visit to determine diagnosis"? 

I am just wondering the best way to meet criteria for 99205. The high risk is usually there (severe exacerbation or chronic illness or SI), but I am not sure how to get the 4 problem points besides ordering labs. The data points are difficult to get in psychiatry.
