How would you define "additional workup planned"? I would consider any testing that was not done withing the confines of the encounter (e.g. lab work, CT scans, patient sent for consultation, etc). Would you count a surgery or an exploratory surgical procedure as additional workup?  I would more than likely count this as a new problem with no addtional workup planned and consider the surgery as a management option.  Your expertise would be greatly appreciated!

I agree with your definition, but I would count exploratory surgery as additional work-up because you are still trying to come up with the diagnosis.  PJ

How about in office procedures, or labs. Such as a rapid strep. Would this be counted as additional work-up?

Would individual therapy sessions, family therapy sessions, etc. be considered additional workup for psych patients?

We recently clarified this with our Medical director and compliance.  If the physician determines that additional labs/x-rays/testing is needed to establish a diagnosis, regardless if the testing is done at the same time as the visit or at a later time, the provider gets "additional work-up" points.  The provider then will determine exactly what tests to order and in Part B of MDM will receive appropriate points for either ordering or reviewing them.

 Referral To: Dr .A    Vascular Surgery
              Reason:LIPOMA EVAL AND REMOVAL

Our organization allows referrals as an additional workup.  If confirmation of a diagnosis or treating a diagnosis that the attending can't treat because it requires a different scope of practice is the reason for the referral, we give them credit for additional workup (hence, a more acute patient).
