Our provider is spending about 15 minutes with pts. and then another 10 minutes documenting.  She then codes a 99214.  I advised that we cannot included documenting as additional time but she states she was advised that she could.  Can someone point me to resource that addresses this.  My understaniong is that documenting the pt encounter is inclusive.ANy advice???

CPT guidelines and both sets of DGs indicate time is "face-to-face".  Site both and show her where it says face-to-face time.
