   Okay  If we had a new patient  has a neck up only exam, is there any other way besides time spent  to qualify for a level 3 office visit.

was the constitutional done (BP, Temp., etc.) done?
if you have eyes: 
pupils, conjunctiva, (2)
neck and thyroid, (2)
exam of tympanics (ears) and nasal mucosa (2)
Lips, teeth , gums (1)
hearing (1)  (if you want to take the time for this)
state the general appearance of the patient (1)
3 of 7 vitals (1)
examine ears,nose (1)
oropharynx - oral mucosa,palates (1)

12 elements in the '97 exam would give you 99203  - providing your hx and medical decision making supports it

Right.  Or if you are using the 95 rules, you probably need 5 - 7 (or sometimes 6 - 7) body areas or organ systems so: Constiutional, EYES, ENMT, Neck, Neuro, lymphatic, skin = 7 = detailed exam.  But as stated correctly above, you would also need a detailed history and at least low complexity MDM.

 well i work for a dermatologist  if that changes things
