I am having a problem coming up with a template of the required elements for a split/shared visit and hope you can help me. If a mid-level sees a patient and so does the physician, what are the required elements for documentation so that it can be billed under the physician's NPI?

The MD must see the patient and document at least one item from the history, exam or MDM.  That's it.  For example, "Patient seen and examined.  Denies chest pain, SOB.  Agree with plan as outlined by PA.

Here, the doc has documented an element of history (ROS) so the visit would be billed to the doctor.

Thank you.  Yes, this I know and understand, I was hoping for something more extensive like a template of sorts.
I feel a little uneasy with this in case of having to defend it to an auditor.
