For the most part, billing for a procedure during a regular office visit is pretty straightforward. As long as you have the documentation to justify it, you add the modifier 25 to the office visit E&M. So if I saw a patient for a visit to address blood pressure and diabetes, but they also had a skin lesion I wanted to biopsy, I would code the visit as:

99213 -25
11100


But what if they came to see me for an annual physical/preventive exam and they had the skin lesion? At our office, I have had some coders tell me to put the modifier 25 on the preventive E&M code and others tell me to not put any modifiers on either. So, which should it be?

99395 -25
11100

OR

99395
11199


Or something else entirely?
