Hello-
I have a question. This patient is an established patient and arrived to the GI office with intermittent rectal bleeding. Apparently it's been going on for 8 months. The note does not indicate "worse" nor "steady continuous bleeding". It only states "patient still has intermittent rectal bleeding."
Intermittent means irregular intervals; not continuous or steady.

Would you consider this as an established problem (stable, improved) which would give it 1 point or would you consider this as an established problem (worsening) which equals 2. The level of the visit matters depending on this. Either 99212 or 99213.

The provider recommended a colonoscopy which it was his previous recommendation 8 months ago.

Your help is appreciated.
